Tomando Control de Su Salud is similar to the Living Well with Chronic Conditions program, except cultural adaptations have been made to the curriculum. Eight out of ten older adults are living with one or more chronic conditions, such as arthritis, cancer, high blood pressure, heart disease, depression or anxiety, lung disease, obesity, etc. Managing your chronic condition(s) can improve your health and quality of life. This program will teach you how to develop strategies to feel your best and to build confidence in your ability to manage your own health through a mutual support from fellow class participants and the class leaders.

About This Class

Tomando Control de Su Salud, also known as the Spanish Chronic Disease Self-Management Program (Spanish CDSMP), was developed at Stanford University and is managed by the Self-Management Resource Center. This class is appropriate for anyone who wants to improve their health and quality of life through managing their chronic condition(s).

This class is taught in Spanish. The class is held once a week for six weeks. Each 2½ hour session includes a combination of presentations, discussions, and activities designed to help participants who are dealing with chronic conditions.

Topics include:

  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Nutrition to support better health
  • Proper use of medication
  • Falls prevention
  • Better breathing techniques
  • Communicating more effectively with family, friends, and healthcare providers
  • Techniques to deal with fatigue, pain, stress, depression, isolation, etc
  • Action-planning, problem-solving, and decision-making

Is there a fee?

The cost for A.C.E. of SW Minnesota to offer Tomando Control de su Salud classes is based on individual voluntary contributions and donations. No one will be denied services because of their inability or unwillingness to contribute towards this service.

Because this service is made possible, in part, under the Federal Older Americans Act through a grant award from the Minnesota River Area Agency on Aging (MNRAAA) under an Area Plan approved by the Minnesota Board on Aging, certain guidelines must be strictly adhered to. Cost-sharing is one of those guidelines. A.C.E. of Southwest Minnesota has a fully established and implemented Cost Share Policy/Statement of Value Process for Title III program participants with coordinating sliding scales.
