Caregiver Coaching (Minnesota model)
What is a Caregiver Coach?
- A coach can help you bridge the gap between knowing what you can be doing in your caregiving role to putting that information into action.
- Considers your needs and identifies issues that are of concern to you.
- Offers support in finding solutions to problems leading you to feel less stressed and more in control.
- Achieves your short and long term goals through the creation of a personalized action plan.
- Connects you with community resources.
- Helps you enhance your ability to provide care to your older family member.
How does coaching work?
You will meet with a coach to go through an interview process (or assessment) that covers the many aspects of caregiving. Your coach will help you recognize issues that are of greatest concern and assist you in identifying strategies and solutions to address those issues. Examples include involving other family members in providing care, receiving education on chronic illnesses or conditions, and learning to take time for self to prevent caregiver burnout. Your coach will communicate with you on a periodic basis to offer support and direction, especially as new issues arise.

A coach will:
- help you in taking specific steps to set goals that enable you to have a satisfying caregiving experience.
- help you to set goals that you might not identify without a coach.
- help prepare you for your role after caregiving.
Is there a fee?
Cost of the A.C.E. Counseling & Caregiver Coach Program is based on income. A sliding fee scale will be used to determine cost of services. No one will be denied services because of their inability to pay for services.
